Tax Receipt Request

Thank you for your support of the Avon Walk for Breast Cancer.

For donations of $250.00 or more that were made offline via check or credit card, receipts are issued by the Avon Foundation for Women the month following the deposit/charge date. For lesser amounts, a canceled check or credit card bill is sufficient under current tax law. If you would still like a receipt for the lesser amount, please use the below form.

If a donation was made online, a receipt was issued via e-mail at the time of the donation to the e-mail address provided at the time of donation. If an e-mail receipt was misplaced for an online donation, please submit the form below to enter the details of your transaction so we may issue a replacement receipt.

All requests should include the full name of the donor, name of Avon Walk participant supported, clarification of which Avon Walk city (Boston, Charlotte, Chicago, Houston, New York, Rocky Mountains, San Francisco, Santa Barbara, Washington DC), the donation amount and date or approximate date of donation. We have also included a space for additional questions and comments.
